Job summary

CircleLink Health is seeking a remote RN Care Coach for part-time work (about 20–25 hours/week) to support Medicare Chronic Care Management. You will coach a panel of patients, provide education, coordinate care, and reduce hospital readmissions from home.

You will work with an experienced team, document thoroughly, manage your schedule, and use our care management software to track outcomes and gaps in preventive care.

Qualifications

• Current, unrestricted Multistate RN license is required.
• Experience with Chronic Care Management and case management preferred.
• Proficiency with EPIC or similar EHR is highly preferred.

Responsibilities

• Call Medicare patients with chronic conditions monthly to manage care.
• Coach patients on SMART goals and self-management strategies.
• Update Plan of Care with medications, appointments, biometrics, and interventions.
• Connect patients to community resources and care transitions.
• Close care gaps with preventive services and follow-up.
