Job summary

CircleLink Health is seeking a remote Part-time RN Care Coach in Illinois to support Medicare Chronic Care Management. You will manage a caseload of patients, provide education, coordinate care, and help close preventive gaps. Requires 3+ years RN experience and IL license, with strong communication and tech skills.

The role offers flexible scheduling, 1099 contract terms, and compensation based on patient encounters, with training provided online.

Qualifications

Current, unrestricted Illinois RN license is required.
Proficiency with EHRs (electronic health records) and web-based applications.
3 or more years' experience as a Registered Nurse.

Responsibilities

Call a full caseload of Medicare patients with two or more chronic conditions on a monthly basis.
Build rapport and coach patients to improved health through SMART goals and education on self-management.
Update Plan of Care by adjusting medications, due/appointments, biometrics, symptoms and interventions.
Connect patients with community resources for transportation, personal care, and prescription/DME assistance.
Conduct Transitional Care Management for high-risk patients discharged from hospital/ER to reduce readmissions.
Close care gaps by promoting preventive care such as annual well visits and vaccines.
