Base pay range

$37.00/hr - $40.00/hr

Contract: 9 Months (with potential for extension)

Pay: $37–$40/hour

Shift Start Times: Vary between 5:00 AM – 12:00 PM (Day Shifts Only but must be open to any start time)

Location: Remote (Must reside in a Compact State or have CA RN License)

Required: Compact license - BSN required

Preferred: Chronic disease management experience in outpatient settings (not required!)

Key Responsibilities

• Act as a care guide and advocate for patients navigating the healthcare system
• Collaborate in patient-centered care team meetings and quality improvement initiatives
• Deliver health and disease education and promote self-management practices
• Support care coordination with internal teams and external healthcare organizations
• Monitor post-hospitalization transitions (admissions, discharges, ER visits)
• Manage care for high-risk patients with chronic conditions and multiple co-morbidities
• Participate in preventive screenings and assist with day-to-day patient interactions
• Promote clear communication among care teams and providers
• Assist with medication management and refills under protocol guidance
• Support clinical audits, reporting, and evaluation of Virtual Care programs
• Collect and analyze biometric and health data for population health initiatives
• Provide clinical support and guidance to Medical Assistants and other care staff

Qualifications

• Required Licensure: Active RN license in a compact state or California (CA) license
• Compact RNs must reside in a compact state
• Willingness to obtain additional licensure as needed (facilitated by employer)

Education: Bachelor of Science in Nursing (BSN) required

Experience:

• 2–5 years in a clinical setting, ideally in outpatient care with chronic disease management
• Experience using Electronic Health Record (EHR) systems
• Proficient with Google Suite and other basic computer tools
