$80,000.00/yr - $100,000.00/yr

Position Title: Remote Case Manager

Position Summary

The Remote Case Manager is responsible for coordinating and managing patient care remotely, ensuring that patients receive comprehensive and continuous care. This role involves assessing patient needs, developing care plans, and facilitating access to necessary services. The Remote Case Manager collaborates with healthcare providers, patients, and their families to achieve optimal health outcomes.

Essential Job Duties

  • Conduct remote assessments of patient needs and develop individualized care plans.
  • Coordinate care and services for patients, including referrals to specialists and community resources.
  • Monitor patient progress and adjust care plans as necessary.
  • Provide patient education and support, including information on treatment options and self-care strategies.
  • Facilitate communication between patients, families, and healthcare providers.
  • Document patient interactions and care plans accurately and timely.
  • Participate in multidisciplinary team meetings to discuss patient care and progress.
  • Utilize telehealth technologies to deliver care and support to patients.
  • Ensure compliance with all relevant regulations and standards.

  • Strong knowledge of case management principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage a remote caseload effectively.
  • Proficiency in using telehealth technologies and electronic health records.
  • Strong organizational and time management skills.
  • Ability to assess patient needs and develop appropriate care plans.
  • Knowledge of community resources and ability to make appropriate referrals.
  • Ability to provide patient education and support remotely.

Education And Experience Required

  • Bachelor's degree in Nursing, Social Work, or a related field.
  • Minimum of two years of experience in case management or a related field.
  • Experience in remote or telehealth case management preferred.
  • Certification in case management (CCM) preferred.

Licenses/Certifications Required

  • Current licensure as a Registered Nurse, Social Worker, or related professional in the state of residence.
  • Certification in case management (CCM) preferred.
