Home Health RN Case Manager Hours

• Class Start Date: June 2. If unable to start by June 2, the candidate will be required to start on July 7.
• 8:00 AM - 5:00 PM EST, Monday to Friday.
• Fully remote position (no onsite requirements).
• Must be Licensed in NY - IF NOT must have Compact License
  
  

Home Health RN Case Manager Compensation

• The pay for this position is $36.00-38.00 an hour
• Benefits are available to full-time employees
• A 401(k) with a company match is available for full-time employees.
  
  

Home Health RN Case Manager Responsibilities

• Providing support to dual eligible populations (Medicare and Medicaid members).
• This role offers a unique opportunity to have a life-changing impact on members enrolled in the Dual Eligible Special Needs Plan (DSNP), who face complex health and social challenges.
• Work closely with members, providers, and community organizations to address the full spectrum of their healthcare and social determinant needs.
• Develop proactive care plans to address both short- and long-term health outcomes, ensuring the overall wellness of members.
• Conduct assessments using clinical tools and review of medical information to evaluate members' needs and benefits.
• Apply clinical judgment to reduce risk factors and address complex health and social barriers impacting care planning.
• Collaborate with interdisciplinary team members, presenting cases at team conferences and contributing to decision-making.
• Utilize case management processes in compliance with regulatory and company policies.
• Engage members using motivational interviewing to ensure maximum participation in their care.
  
  

Home Health RN Case Manager Requirements

• RN License required
• 3-5 years of clinical experience required, with 2-3 years of case management (CM), discharge planning, and/or home health coordination experience.
• Bilingual Candidates Preferred
