Job summary

Sylvan Health is seeking a mission-driven registered dietitian to provide telehealth nutrition therapy and evidence-based care in a remote setting. You will conduct personalized consultations, monitor patient progress, and collaborate with the care team to optimize outcomes for patients with complex chronic conditions.

The role emphasizes empathy, accurate documentation in EMR, and the ability to adapt in a fast-paced environment while delivering high-quality coaching and education to patients

Qualifications

• Licensure in TN, IL, PA preferred
• Bachelor's Degree in Nutrition/Dietetics required
• Completion of Accredited Dietetic Internship required
• Credentialed by the Commission on Dietetic Registration (CDR) required
• Minimum 1–2 years of post-internship nutrition experience
• Experience counseling patients virtually or in an outpatient setting
• Experience treating GI conditions such as IBD, IBS, fatty liver, celiac disease, etc.
• Comfort educating patients with complex chronic conditions (diabetes, kidney disease)

Responsibilities

• Conduct personalized nutrition therapy consultations via telehealth
• Administer evidence-based clinical protocols and provide individualized nutrition support
• Monitor patient progress and adjust care plans to improve outcomes
• Aim for high patient retention by following program guidelines
• Collaborate with care team members
• Utilize meal planning tools to support patient adherence
• Participate in clinical rounds with partner practices and care teams
• Attend quarterly RD group meetings and monthly 1:1s with CNM
• Support patients in making lifestyle changes
• Build trust with patients and caregivers quickly
• Document clinical information in an Electronic Medical Record
• Adapt to fast-paced, evolving environments while maintaining a positive attitude
• Transition between patients efficiently following clinical procedures
