Job summary

Net Health is seeking a licensed Physical Therapy Assistant to join as a Care Navigator in a remote, full-time capacity. You will support patient success by guiding home-exercise programs, tracking progress, and communicating with clinicians to optimize care.

The ideal candidate has PTA credentials, musculoskeletal experience, and strong multi-platform skills (EHR, scheduling, messaging). A focus on empathy, patient outcomes, and data-driven decisions is essential.

Qualifications

• PTA with musculoskeletal health experience is preferred.
• Must have graduated from an accredited physical therapy program.
• Tech-savvy; comfortable using multiple platforms (EHR, messaging, scheduling).
• Strong communication, interpersonal, and data-presentation skills to drive decisions.

Responsibilities

• Provide remote monitoring to support patients in home-exercise therapy.
• Direct and assist patients in therapeutic exercise, neuromuscular re-education, and functional training via our platform.
• Provide feedback to the clinician on patient progress outside the clinic.
• Maintain records and notes; help improve platform processes and functionality.
• Manage a caseload efficiently and work cross-functionally.
