Job summary

Finger Lakes Community Health seeks a Psychiatric Care Manager to coordinate and support psychiatric services across our health centers. The role manages referrals, facilitates communication among patients, providers, and behavioral health professionals, and develops individualized treatment plans with the integrated care team.

Qualifications

• 3+ years in psychiatric/behavioral health care as SME and able to work independently
• Supervisory experience preferred
• Valid government-issued photo ID required
• Current NYS RN license required
• CPR certification required
• Continuing education per NYS licensing renewal requirements
• Strong knowledge of psychiatric disorders and treatments
• Ability to lead discussions across departments and facilitate team communication
• Ability to initiate value-adding improvements

Responsibilities

• Oversee psychiatric referrals from intake to treatment ensuring timely access and coordination
• Coordinate care among prescribers, primary care providers, social workers, therapists, patients, families and community resources
• Support crisis management and immediate interven­tions for patients in crisis
• Educate patients and families about psychiatric conditions, medications and coping strategies
• Participate in quality improvement to enhance care coordination and outcomes
• Document all patient interactions and care in the EHR per policies
• Schedule and supervise psychiatry team for adequate coverage
• Perform other duties as assigned
