Qualifications

• PharmD or Bachelor of Pharmacy required.
• Active, unrestricted U.S. pharmacist license (multi-state preferred).
• Experience in hospital, ambulatory, community, or clinical pharmacy valued.

Responsibilities

• Review prior authorization requests for clinical appropriateness and plan criteria.
• Apply guidelines, formulary rules, and regulatory standards to approve/deny requests.
• Communicate determinations to providers and members with documentation.
• Collaborate with medical directors and clinical teams on complex cases.
• Document decisions in compliance with CMS, state, and health-plan requirements.
• Support process improvements to reduce turnaround times and improve member experience.
