Job summary

CareOregon, Inc. is seeking a Registered Nurse to provide remote primary care support. You will triage calls, assess discharge needs, manage medications, and counsel patients and caregivers while collaborating with the care team.

The role emphasizes nursing judgment, adherence to Oregon regulations, and thorough documentation in the EHR. Ideal candidates hold an RN license in Oregon with at least two years in primary care or related settings and are capable of working independently while

Qualifications

• Associate degree or higher in Nursing (or higher).
• Current unencumbered RN license in Oregon.
• Minimum 2 years in nursing related to primary care, geriatrics, home health or hospice.

Responsibilities

• Triage patient calls/faxes/requests and develop a care plan using protocols.
• Make outbound calls post-hospitalization to assess and plan for discharge needs.
• Evaluate and assist with medication needs, including refills and prior authorizations.
• Provide disease-specific teaching and counseling to patients and caregivers.
• Communicate lab results and educate on implications.
• Collaborate with interdisciplinary team to implement comprehensive care plans.
• Apply nursing process and evidence-based guidelines in patient care.
• Document all patient communications in the EHR.
• Assist with medication reconciliation and transitions in care.
