Job summary

Moda Health seeks a dedicated coordinator to support the Diabetes Prevention Program, coordinating enrollment, outreach, and data documentation for CDC reporting.

The role emphasizes collaboration across departments, accurate data submission, and monitoring KPIs such as enrollment, attendance, and outcomes. Strong communication, DEI understanding, and health promotion background are preferred. Flexibility to work early mornings or evenings may be required to meet participant needs.

Qualifications

• Bachelor's degree in health promotion preferred or equivalent healthcare experience.
• Experience in program support and coordinating multiple projects with extended timelines in healthcare operations.
• Ability to work collaboratively and independently.
• Excellent communication, interpersonal, and presentation skills.
• Strong cultural competence and DEI understanding to guide work.
• Knowledge of health behavior change theory.
• Experience with proprietary databases and maintaining confidentiality.

Responsibilities

• Coordinate and implement Diabetes Prevention Program.
• Outreach and enrollment of new members via phone, email, mail and virtual methods.
• Assist care team with clerical support and program operations.
• Document member progress and submit data to CDC and internal committees.
• Ensure data is submitted accurately and timely with data team.
• Develop and monitor KPIs including outreach, enrollment, attendance, retention, and health outcomes.
• Manage program calendar, emails, and phone lines.
• Adhere to privacy and data security regulations.
