Qualifications

• High School Diploma or GED with healthcare experience or a specialized medical assistant diploma.
• 3 years experience in a complex health system environment or less than 1 year with specialized diploma.
• Proficient in Microsoft Office applications.
• Analytical thinking and ability to handle multiple tasks.
• Superior customer service and professional communication.
• Working knowledge of basic computer functionality and privacy.
• Ability to adapt to changing priorities and think critically.
• Knowledge of medical terminology and EMR/scheduling software.

Responsibilities

• Leads setup and ongoing management of post-discharge and care gap reports.
• Interacts with payer groups and leadership to drive patient engagement strategies.
• Collaborates with nursing to review barriers to care coordination.
• Coordinates patient engagement guidelines for referrals to CM services.
• Creates and maintains quality data and spreadsheets for reporting.
