Your Health Organization in the Grandstrand area is seeking a Pharmacist to support patients across facilities and homes through medication management, patient consultations, and care coordination. The role emphasizes safe, evidence-based therapy and reducing rehospitalizations via MTM and telehealth visits.

Qualifications

• PharmD degree required.
• Licensed Pharmacist in good standing.
• Clinical pharmacy experience preferred.
• Strong clinical knowledge and critical thinking for patient care.
• Ability to coordinate care and manage multiple responsibilities.

Responsibilities

• Ensure accurate dispensing, compounding, and distribution of medications per regulations.
• Counsel patients on proper use, dosage, and potential side effects.
• Monitor therapy outcomes and suggest adjustments as needed.
• Provide MTM services including reviews, reconciliation, and adherence assessment.
• Conduct telehealth or in-person consultations to address concerns and optimize plans.
• Collaborate with care teams to develop comprehensive care plans for high‑risk patients.
• Document encounters and interventions in EHR systems; generate reports for quality improvement.
