Job summary

PharMerica, part of BrightSpring Health Services, is seeking a Pharmacist Consultant to support long-term care facilities across Hartford CT and Springfield MA. This remote role requires an immunization certification and licensure, with weekend availability as needed, to optimize medication therapy and ensure regulatory compliance.

Qualifications

• Bachelor's degree or PharmD from an accredited school.
• Current state license; dual license preferred.
• Experience in acute, long-term care or home health preferred.

Responsibilities

• Evaluates medication orders for appropriateness and makes recommendations to physician and staff.
• Reviews residents' drug regimens monthly and reports findings to Director of Nursing and physicians.
• Documents all communications with physicians, nurses, and families.
• Maintains knowledge of pharmacy laws and regulations for long-term care.
• Works cooperatively as a team and maintains knowledge of pharmaceutical care for residents.
