Job summary
PharMerica is seeking a Pharmacist Consultant to support long‑term care and senior living clients. This remote role covers Modesto, Fresno and Stockton, CA, and requires an Immunization Certification and a current pharmacist license.

Qualifications
- Pharmacy degree from accredited school with active license.
- Experience in long-term care or home health is preferred.
- Ability to work weekends or outside normal hours when needed.
Responsibilities
- Evaluates physician medication orders for appropriateness and recommends therapy changes.
- Reviews resident drug regimens monthly and reports findings with recommendations.
- Documents communications with physicians, nurses, and residents/families.
- Maintains knowledge of pharmacy laws and long-term care regulations.
- Works cooperatively as part of a care team.
- Keeps up to date with pharmaceutical care practices for residents.
