Actalent is seeking a Remote Clinical Pharmacist MTM Case Manager to support Missouri Medicaid members from a fully remote location. You will conduct MTM reviews, educate patients, and develop care plans while collaborating with providers and community partners.

Requires active Missouri Pharmacist License and Missouri residency. The role offers a remote work schedule with a focus on clinical care over volume, and includes eligibility for standard benefits for a contract position.

Responsibilities

• Identify and resolve medication-related concerns and barriers to treatment.
• Collaborate with physicians, pharmacies, caregivers, case managers, and community health partners.
• Assess patient needs and help connect them to appropriate healthcare and community resources.
• Support patients managing chronic conditions and complex medication regimens.
• Maintain accurate clinical documentation, including SOAP notes.
• Serve as a trusted healthcare resource for patients who may have limited access to care.

Job Type & Location

Pay and Benefits

The pay range for this position is $50.00 - $55.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
