Job summary

Qualifications

• Bachelor's Degree in Nursing required or Master's Degree preferred.
• MA Registered Nurse License required.
• 2-3 years of experience in acute care, provider practice, or discharge planning preferred.

Responsibilities

• Collaborate with interdisciplinary team to tailor a health care plan for assigned patients.
• Develop an individualized plan with the MD for appropriate services for the patient.
• Report pertinent observations regarding clients to the primary care provider, social worker, or Director.
• Coordinate discharge planning needs with the healthcare team.
• Resolve patient problems using multidisciplinary strategies.
• Educate patients on navigating the health system.
