Job summary

Mass General Brigham in Boston, MA is seeking a Utilization Management RN to coordinate complex care needs and serve as the primary contact for service access and collaboration with the care team.

You will develop individualized care and discharge plans with the physician, report observations to the team, and educate patients to navigate the health system. A Bachelor's in Nursing is required, with a Master's preferred, and MA RN licensure.

Qualifications

• Bachelor's Degree in Nursing required; Master's in Nursing preferred.
• 2-3 years in an acute care setting and/or provider practice and/or community-based role with discharge/transitional planning background preferred.
• MA Registered Nurse License required.

Responsibilities

• Collaborate with the interdisciplinary team to create tailored health care plans for assigned patients.
• Develop individualized plans with the MD for appropriate services for the patient.
• Report pertinent observations and reactions regarding clients to the appropriate person.
• Assess and coordinate patient's discharge planning needs with members of the healthcare team.
• Resolve patient problems and needs by utilizing multidisciplinary team strategies.
• Educate patients on navigating throughout the health system.
