Job summary

Included Health is seeking a Newborn/Pediatric Nurse Care Manager to guide members through complex medical and behavioral health situations via telephonic and video care. You will collaborate with a multidisciplinary team to develop cohesive care plans and advocate for patients, ensuring longitudinal, high-quality care.

The role emphasizes remote work, HIPAA compliance, and empathy-driven support for families navigating diverse medical needs while achieving volume and quality targets.

Qualifications

• BSN required and active RN license in good standing.
• Must reside in a compact NLC state.
• Willingness to obtain licensure in multiple states.
• CCM (Certified Case Manager) certification preferred.
• Minimum of 5 years of nursing experience.
• 2+ years in Pediatric setting.
• 2+ years in care, case and disease management (health plan/TPA) experience.
• Be able to work until 6pm PST.

Responsibilities

• Deliver coordinated, patient-centered virtual Care Management by telephone and/or video.
• Generate care plans with members and multidisciplinary team to achieve goals.
• Help navigate complex medical conditions, benefits, and the healthcare system.
• Partner with members’ local providers to ensure coordinated care.
• Provide compassionate, longitudinal follow-up care and build relationships.
• Assist during hospitalizations and rehabilitation stays with coordinated Case Management.
• Coordinate necessary resources addressing clinical or social needs.
