Job summary

Imagine Pediatrics is seeking a Resource Specialist Care Manager to support families of medically complex children. You will provide emotional support, care coordination, and resourcing assistance using an integrated platform with a full interdisciplinary team including physicians, RNs, therapists, and navigators.

Travel up to 10% for offsite meetings, training, and education. Hours are 8:00am–5:00pm EST with some shifts 10:00am–7:00pm EST, Monday–Friday.

Qualifications

• 2-3+ years postgrad experience in social service case management
• Minimum 1 year of experience working with pediatric population
• Knowledge and application of motivational interviewing practices
• Experience with telehealth services and remote family engagement
• Bilingual Spanish strongly preferred

Responsibilities

• Conduct thorough assessments of medical, behavioral, and social needs of medically complex children
• Identify barriers to care such as housing, transportation, and insurance
• Collaborate with families and multidisciplinary teams to set individualized care goals
• Develop and maintain integrated care plans addressing medical, behavioral, and social needs
• Ensure timely follow-up on referrals, evaluations, and treatment recommendations
• Provide education to families about navigating local resources and agencies
• Build safety plans for caregiver health needs and domestic violence
• Advocate for resources and connect families with available services
• Represent the organization at community events and resource meetings
