Candidate must be licensed in the one of the following states: NY, NJ, NC, GA, FL, TX, MO, TN or Washington DC.
10 % travel for offsite meetings, training, and educational purposes.
8:00am-5:00pm EST Monday-Friday with 2 days determined with supervisor being 10:00am-7:00pm EST.
As a Resource Specialist Care Manager, you will work with the families of medically complex children providing emotional support, care coordination, and resourcing support in navigating the healthcare system. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including physicians, RNs, care team assistants, mental health therapists, community health navigators, pharmacists, dietitians, and more. The salary range for this position ranges from $70,000-77,000 annually in addition to competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable). When determining compensation, we analyze and carefully consider several factors including job-related knowledge, skills and experience. These considerations may cause your compensation to vary.
