Bayfrontstpete is seeking an RN for Remote Patient Monitoring (RPM) to deliver expert virtual nursing care for patients with chronic conditions. The role supports a virtual-first care model and offers day shift, part-time on weekends.

You will onboard patients, educate on monitoring devices, build personalized care plans, review biometric data, and coordinate across care teams to optimize outcomes and reduce avoidable utilization.

Qualificações

• RN with Florida license; BSN preferred per posting.
• Ambulatory care and cardiac experience preferred; multi-specialty is acceptable.
• BLS certification current or obtained within 90 days.

Responsabilidades

• Deliver virtual nursing care to patients with chronic conditions.
• Onboard patients and educate on RPM devices and data transmission.
• Develop and update personalized care plans using biometric trends.
• Coordinate care and referrals to address barriers.
• Document patient interactions in EHR and ensure data integrity.
