Job summary

Hemophilia of Georgia seeks an Outreach Nurse to educate, assess, and coordinate services for HoG clients and families. The role integrates with the hemophilia treatment team, providing consultation to schools and clinics and supporting telehealth and home visits.

Responsibilities include home assessments, care planning, documentation, and collaboration with social workers. Travel and occasional evening/weekend hours are required as part of program coverage.

Qualifications

• Nursing degree from an accredited nursing program.
• Registered Nurse license in the state of Georgia.
• A minimum of three years’ experience providing clinical care.
• Demonstrated clinical competence in infusion skills.
• Self directed; ability to work independently; ability to multi-task.
• Ability to prioritize work assignments and manage time efficiently.

Responsibilities

• Develop and maintain a sound knowledge base in all facets of hemophilia and other bleeding disorders, the nature of the disorders, treatment, complications and their treatments, client population, aspects of HoG, Georgia resources, etc.
• Analyze and review caseload of clients as assigned by the Director of Nursing and the VP of Programs; determine eligibility for HoG programs and provide services.
• Visit client/family in home setting for assessment and education, including home therapy training.
• Document all encounters with clients in required nursing documentation.
• Assist client in preparing a plan to address emergencies.
• Educate the client's medical and community support system (emergency room, physicians, schools).
• Coordinate care, including crisis intervention, home care, and referrals as needed.
