Zusammenfassung

Elevance Health’s CareBridge Health team is seeking an Outreach Care Specialist to ensure member treatment plans are followed on less complex cases and to proactively improve member health outcomes. The role is remote with required in-person training sessions and occasional office presence per policy, offering flexible hours and autonomy.

Responsibilities include coordinating referrals, scheduling follow-ups, assessing treatment plan adherence, and identifying barriers to care.

Qualifikationen

• Requires a HS diploma or equivalent and at least 1 year of related experience.
• Equivalent combination of education and experience will be considered.

Aufgaben

• Initiates and manages clinical referrals and orders (specialists, labs, imaging).
• Coordinates follow-up care by scheduling appointments or enrolling in programs.
• Assesses member compliance with treatment plans via phone or on-site visits.
• Identifies barriers to plan compliance and coordinates resolutions.
• Recommends treatment plan modifications with case management and provider.
• Coaches members on ways to reduce health risks.
• Prepares reports documenting case and compliance updates.
• Participates in cross-functional teams on projects and process improvements.
