Job summary

Mercy Clinic Orthopedics Southfork is seeking a compassionate Registered Nurse (RN) Nurse Navigator to guide orthopedic patients through evaluation, surgery, recovery, and follow-up care. You will coordinate care, provide education, and support patients from initial consultation to discharge and beyond.

Working with orthopedic surgeons and interdisciplinary teams, you will ensure timely, high-quality care while delivering an exceptional patient experience.

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license in the state of practice.
• 2-3 years of nursing experience.
• Basic Life Support (BLS) certification.

Responsibilities

• Coordinate care for orthopedic patients throughout evaluation, treatment, surgery, and recovery.
• Educate patients and families regarding diagnoses, procedures, treatment plans, and post-operative care.
• Facilitate communication between patients, providers, and care teams to ensure seamless care coordination.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
• Monitor patient progress and address barriers to care.
• Maintain accurate and timely documentation in EPIC and other clinical systems.
• Track outcomes and quality metrics to support evidence-based practice and improved patient outcomes.
• Participate in quality improvement initiatives and program development efforts.
• Serve as a clinical resource for patients, families, providers, and staff.
