Job summary

Cardinal Health is seeking a Nurse, Individualized Care Nurse Care Management to remotely coordinate structured, patient‑centered care for patients with complex chronic diseases, including cancer. You will bridge gaps between patients, providers and support services to promote continuity of care.

You will manage a caseload of 100–150 members, perform telephonic care management, document thoroughly in the EHR, and collaborate with onsite and remote teams to optimize outcomes and adherence.

Qualifications

• Active RN license in the United States
• 5+ years hospital or clinical experience with complex chronic diseases
• Oncology patient experience preferred; care management background beneficial

Responsibilities

• Coordinate care across care management and oncology teams to support patient outcomes
• Review patient needs, document in EHR, and ensure timely follow‑ups and care transitions
• Perform medication reconciliation and reinforce self‑management education
• Collaborate with interdisciplinary teams to develop and adjust care plans as needed
