Job summary

Altea Healthcare in Ohio is seeking a part-time Certified Nurse Practitioner (NP) to join our post-acute care team. You will be taking remote calls for CCM visits at a national level, with weekend coverage from 7:00 AM to 7:00 PM or 7:00 PM to 7:00 AM; Ohio license required before start date, and willingness to obtain additional state licenses as needed.

The schedule is varied, including holidays, weekends, and evenings.

Qualifications

• Active NP license in Ohio required prior to start.
• DEA registration or willingness to obtain prior to start.
• Experience in geriatrics, internal medicine, or primary care.
• Willingness to obtain additional state licenses as needed.
• Weekend and holiday/evening availability.

Responsibilities

• Examine patients' medical records
• Ordering and interpreting diagnostic tests (labs, imaging, etc.).
• Diagnosing and managing acute and chronic conditions.
• Prescribing medications and creating treatment plans.
• Collaborating with physicians, nurses, and facility staff.
• Educating patients and families on health conditions and preventive care.
• Documenting patient care accurately and efficiently.
• Oversee patients’ overall health
• Promote good health practices
• Other duties as assigned

What You’ll Do:

As a NP, you will be taking calls remotely for national level for CCM visits from weekend coverage from 7:00 AM to 7:00 PM or 7:00 PM to 7:00 AM, rotating with our other providers. These duties can be performed remotely but you must be available to take the calls when you are scheduled to work. The ideal candidate will hold an active Ohio license to conduct monthly CCM visits and be willing to obtain additional state licenses as needed. Typical schedule may be varied and shall include holidays, weekends, and evenings.

You should have experience in one of the following specialties: acute care, family health or geriatrics. Patience, problem-solving, and good communication skills are important.
