Job summary

Altea Healthcare in Ohio is seeking a Certified Nurse Practitioner to deliver post-acute care in a skilled nursing setting. You will diagnose, treat, and guide patients while collaborating with the care team to ensure optimal outcomes.

The role is part-time with a weekend schedule and mandatory travel between Toledo and Oregon facilities. Training and mentorship are provided to support both new grads and experienced providers in delivering high-quality care.

Qualifications

• Must be a Certified Nurse Practitioner with active or eligible NP license.
• Experience in geriatrics, post-acute, or primary care preferred.
• Strong communication and teamwork skills required.

Responsibilities

• Perform physical exams and review medical histories.
• Order and interpret diagnostics as needed.
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and create treatment plans.
• Collaborate with physicians, nurses, and facility staff.
• Educate patients and families on health and preventive care.

What You’ll Do:

As a Certified NP, you will be a key clinical provider in a skilled nursing facility, diagnosing, treating, and guiding patients to better health. No two days are the same!

Your daily responsibilities include:

• Performing physical exams and reviewing medical histories.
• Ordering and interpreting diagnostic tests (labs, imaging, etc.).
• Diagnosing and managing acute and chronic conditions.
• Prescribing medications and creating treatment plans.
• Collaborating with physicians, nurses, and facility staff.
• Educating patients and families on health conditions and preventive care.
• Documenting patient care accurately and efficiently.
