Job summary

The Jean Tweed Centre is seeking a Nurse Practitioner to deliver non-urgent primary care assessments via in-person or telemedicine. The role focuses on continuity of care and supportive transitions for clients with substance use and mental health needs, with emphasis on trauma-informed, client-centred practice.

The remote position offers up to 30 hours per week, typically daytime, Monday to Friday, with four days per month covering admission cycles.

Qualifications

• Bachelor’s degree with Advanced Preparation in Primary Care or Adult Nursing.
• Basic Cardiac Life Support certification.
• Registered with College of Nurses of Ontario; RN-Extended Class.
• 2–4 years of clinical experience in hospital or community health.
• Experience with substance use, mental health, and gambling care preferred.

Responsibilities

• Provide direct clinical assessments and primary care within RN extended scope.
• Use telemedicine to assess clients and determine care needs.
• Collaborate with interprofessional team for care planning and follow-up.
• Educate and coach staff and students to improve problem solving.
• Support data collection and reporting for program indicators.
