Job summary

Tembo Health is seeking a Family/Adult-Gerontology Nurse Practitioner to join our Illinois market as the first full-time remote clinician focused on dementia care navigation. You will conduct virtual dementia assessments, review medications, and develop care plans in collaboration with care navigators.

You will work with our head of medical to refine workflows, support medical escalations, and help scale our GUIDE program.

Qualifications

• Licensed as a Family or Adult-Gerontology Nurse Practitioner in Illinois.
• Minimum 1 year experience as NP.
• Licensed to practice virtual/outpatient care in any state.
• Must not be excluded from participating in any state or federal healthcare programs.
• Passion for caring for patients with dementia.
• Comfort with learning new technology systems.

Responsibilities

• Conduct virtual assessments of patients with suspected dementia.
• Review medications to reduce high-risk meds and start helpful therapies.
• Develop comprehensive care plans with care navigators.
• Coordinate care with primary teams and external providers.
• Support urgent care and other geriatric needs as the practice evolves.
