Job summary

Remo Health, Inc. is seeking a remote Advanced Practice Clinician (PMHNP or AGPCNP) to lead dementia assessment and longitudinal care. You will work with an interdisciplinary team to support patients with cognitive disorders and their caregivers in a startup-driven, digital health environment.

The role emphasizes dyad-centered care, telehealth delivery, and continuous improvement of the care model and platform. Licensure in OR/WA/OH/ID is required.

Qualifications

• Board-certified PMHNP-BC or AGPCNP-BC with active US license.
• Graduate of an accredited US nursing school and NP program.
• Experience in geriatrics, memory care or dementia.
• Strong communication with patients and caregivers.
• Proven ability to work within an interdisciplinary care team.

Responsibilities

• Perform diagnostic evaluations to establish and stage dementia diagnoses.
• Drive care plan formulation with evidence-based goals.
• Provide longitudinal, dyad-centered care for patients and caregivers.
• Collaborate with Neurologists, Geriatricians, Care Navigators and other team members.
• Support 24/7 care access through structured workflows.
