Qualifications

• Certification as a Case Manager is preferred; BS in health/human services preferred.

• Perform care management within licensure scope for complex cases.
• Assess, develop, implement, coordinate, monitor and evaluate care plans.
• Coordinate resources and communicate with Medical Directors and providers.
