Job summary

Elevance Health is seeking a Nurse Case Manager I in Latham, NY for telephonic or on-site care management of members with complex needs. The role offers hybrid/virtual work, with in-person onboarding and training sessions as needed.

You will assess, develop, implement, coordinate, monitor, and evaluate care plans across the care continuum. Minimum: BA/BS in a health-related field and 3+ years clinical experience; RN license in applicable states; multistate licensure required.

Qualifications

• Requires BA/BS in a health-related field and minimum 3 years of clinical experience.
• Current, unrestricted RN license in applicable state(s).
• Multi-state licensure required if providing services in multiple states.

Responsibilities

• Ensure member access to services appropriate to health needs.
• Conduct assessments to identify needs and develop care management plans.
• Coordinate authorizations/referrals within benefits structure and external arrangements.
• Collaborate with internal and external resources to meet identified needs.
• Monitor and evaluate the care management plan effectiveness.
