Job summary

Cardinal Health is seeking a Nurse, Individualized Care Nurse Care Management to coordinate medical treatments, deliver patient education, and bridge gaps between patients, providers, and support systems. You will manage a caseload of 100–150 members and work remotely full-time.

The role emphasizes patient-centered care, collaboration with the oncology care team, and adherence to HIPAA. A California RN license and strong organizational skills are required.

Qualifications

• RN with current, unrestricted license (CA) preferred or required
• 5 years hospital/clinical experience with complex chronic disease
• Care management experience strongly preferred
• Oncology patient experience strongly preferred
• Superior communication skills, verbal and written

Responsibilities

• Collaborate with health care staff to develop, implement, monitor and evaluate care plans
• Manage a caseload (100–150 members) and coordinate care transitions
• Document encounters in electronic health records with accuracy
• Coordinate with oncology team and care management for patient care
• Attend meetings, provide patient education and on-going care coordination
