Cardinal Health is seeking a Nurse, Individualized Care Manager to coordinate medical treatments, provide patient education, and bridge gaps between patients and providers. You will manage a high-volume caseload and work remotely with an on-site team when needed.

Qualifications include California RN license and 5 years hospital/clinical experience; oncology care is preferred. The role emphasizes HIPAA compliance, documentation accuracy, and cross-functional collaboration within Clinical

Qualifications

• Registered Nurse with a current, unrestricted California license.
• 5 years’ hospital/clinical experience with complex chronic diseases preferred.
• Care Management experience strongly preferred.
• Oncology patient experience strongly preferred.
• Experience with Microsoft Office products.
• Superior communication skills (verbal and written).
• Ability to work collaboratively; team focused.
• Excellent organizational skills.
• Bilingual skills a plus.

Responsibilities

• Collaborate with health care staff to develop, implement, monitor and evaluate clinical care and coordinate care management.
• Manage care for patients with chronic disease states, including cancer.
• Ensure documentation compliance with federal, state and regulatory agencies.
• Document encounters accurately in the designated system and ensure timely updates.
• Participate in interdisciplinary case conferences and team huddles for coordinated care.
• Identify patients for case management, assess needs, and coordinate care modifications.
• Develop and review clinical pathway trends and share with teams.
• Assist with quality improvement by identifying trends and barriers.
