You will guide members through complex medical and behavioral health situations by telephone or video, collaborating with a diverse clinical team to deliver integrated remote care. The role requires a CA RN license, CCM certification, 5+ years nursing, 2+ years in care management, and residency in California.

Qualifications

• Bachelor of Science in Nursing required.
• Must have CCM certification or eligible for exam by Aug 2025.
• Active California Nursing License required.
• 5+ years nursing experience and 2+ years in care management.
• Must reside in California and be based in CA.

Responsibilities

• Deliver coordinated, patient-centered virtual care management by telephone or video.
• Create care plans with members and care team to meet goals.
• Navigate complex medical conditions, benefits, and healthcare system.
• Coordinate with members’ local providers for coordinated care.
• Provide longitudinal follow-up care and compassionate support.
• Assist during acute healthcare episodes with case management.
• Coordinate resources to address clinical and social needs.
