Qualifications

• Active Registered Nurse license in your state of residence.
• At least five years of healthcare experience.
• Experience in case management, care coordination, patient navigation, or related clinical environments.
• Ability to work remotely and manage a caseload independently.
• Strong communication and clinical judgment.

Responsibilities

• Educate clients and advocate on their behalf to help obtain high-quality care.
• Coordinate care and provide network provider recommendations.
• Educate and support clients through transitions across PHM's clinical and operational teams.
• Collaborate with client services coordinators and medical records to holistically manage cases.
