The role emphasizes timely access to diagnostics and coordinated care between visits, including telehealth and urgent care coverage.

Qualifications

• Completion of NP or PA program and licensure in California.
• Two years’ experience delivering primary care as NP/PA or NP residency completion.
• Board certification for NP or PA and willingness to meet credentialing requirements.
• Experience with NCQA Patient-Centered Medical Home or Joint Commission accreditation preferred.

Responsibilities

• Provide patient visits via telehealth to review urgent or timely diagnostic results.
• Support primary care teams with lab reviews, imaging results, and follow-up communication.
• Serve as backup for urgent questions and medication review across sites.
• Collaborate in scheduling and coordination to achieve timely care.
