Qualifications

• Current and unrestricted NP license required.
• Experience providing telephonic clinical coverage in post-acute care settings.
• Understanding of SNF operations and quality measures.

Responsibilities

• Lead clinical oversight of call solution clinician functions.
• Direct monthly meetings and group discussions with RMD/VPO.
• Interact with leadership to mentor clinicians and improve care quality.
• Ensure timely documentation, scheduling follow-ups, and patient safety.
