Job summary

Alumus is seeking a Nurse Practitioner to deliver home-based primary care with telehealth support. You will manage chronic and acute conditions, perform assessments, and collaborate with facilities to optimize patient outcomes.

Qualifications

• Master’s degree in Nursing or RN license.
• Advanced Practice Nursing license.
• NP Board Certification from recognized certifying body.
• Fingerprint clearance.
• Unrestricted prescribing and dispensing privileges with DEA licensure required.
• 1–2 years NP experience preferred
• Experience with adult and geriatric populations and population health management preferred
• Knowledge of Skilled Nursing/Transition Rehabilitation Centers (preferred)
• Ability to work with confidential information including HIPAA protected information
• Ability to be proactive and initiate processes with little to no supervision

Responsibilities

• Perform comprehensive patient assessments and collaborate with facility staff on preventive services and early changes in condition recognition.
• Manage acute and chronic conditions, including medication adjustments.
• Order and interpret diagnostic tests.
• Develop, implement, and modify treatment plans as needed.
• Participate in quality assurance and improvement activities.
• Provide clinical education and training for patients, families, and facility staff.
• Adhere to state-specific NP scope of practice requirements.
• Demonstrates knowledge of etiologies, risk factors, pathophysiology, and presentation of acute and chronic conditions
