Responsibilities

• Engage members via telephone to assess needs across medical/behavioral/social domains.
• Develop and implement person-centered care plans for chronic conditions and social determinants of health.
• Collaborate with care team and providers to execute care plans.
• Educate and coach members for self-management and healthy lifestyle changes.
• Support discharge planning and care transitions.
• Advocate for member needs within the care team.
