Job summary

UnitedHealthcare is seeking a NICU Registered Nurse Case Manager to liaison with Medicaid families, coordinating medical, behavioral, and socioeconomic needs to ensure access to appropriate care. The role involves home and social assessments, education, and patient advocacy, with emphasis on complex cases and cross-team collaboration.

WA residents, especially in the Seattle/Tacoma area, may work remotely while handling challenging cases, with a focus on improving health outcomes and service

Qualifications

• Current, unrestricted RN license in Washington state.
• 2+ years NICU clinical experience.
• 1+ year experience with MS Office (Word, Excel, Outlook, Teams).
• Reside in the state of Washington.
• Designated quiet workspace with access to secure high-speed home internet.

Responsibilities

• Engage members telephonically to complete needs assessments across medical, behavioral, functional, cultural, and socioeconomic dimensions.
• Develop and implement person-centered care plans for chronic conditions and social determinants of health.
• Collaborate with internal care teams, providers, and community resources to execute care plans.
• Provide education and coaching to support member self-management and healthy lifestyles.
• Support proactive discharge planning and care transition coordination.
• Advocate for members and families to ensure needs and choices are represented.
