Job summary

Elevance Health is seeking a Telephonic NICU Nurse Case Manager II in New York. The role is virtual with required in-person training and some office-site duties as needed. You will assess and manage complex chronic cases, coordinating care across the continuum to improve outcomes.

Requires an RN license, BA/BS in a health field, and 5+ years clinical experience; multi-state licensure is needed if practicing in several states. Benefits include comprehensive packages and 401k contributions.

Qualifications

• Requires BA/BS in a health-related field and at least 5 years of clinical experience.
• Current, unrestricted RN license in applicable state(s) required.
• Multi-state licensure is required if providing services in multiple states.

Responsibilities

• Assess, develop, implement, coordinate, monitor, and evaluate care plans to optimize member health care.
• Conduct care management tasks telephonically or on-site, including discharge planning.
• Coordinate internal and external resources to meet identified needs.
• Monitor and evaluate effectiveness of care plans and adjust as necessary.
• Interface with Medical Directors and Physician Advisors on care plans.
• Negotiate rates of reimbursement where applicable.
• Assist with problem solving with providers, claims or service issues.
• Support development of utilization/care management policies and procedures.
