Local Infusion is seeking a licensed Nurse Practitioner in Massachusetts to join our remote, patient-facing neurology and memory care team. The role emphasizes telehealth assessments, care planning, and collaboration with physicians to improve access and timely treatment for patients with cognitive disorders.

Work with neurologists and care team to educate patients and families, document accurately in EMR, and manage a full patient panel with supervision as needed.

Qualifications

• Active, unrestricted NP license in Massachusetts; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgement and clinical documentation skills
• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers

Responsabilités

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies
• Partner with our neurologist(s) and broader care team to ensure continuity of care
• Educate patients and families on diagnoses, treatment plans, and disease progression
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
