Job summary

Local Infusion is seeking a licensed Nurse Practitioner in Massachusetts for a remote, patient-facing role. You will deliver telehealth care to patients with neurological conditions and memory disorders, partnering with physicians to improve access and reduce time to treatment.

You will conduct assessments, develop care plans, and prescribe therapies as needed, while educating patients and families. Thorough EMR documentation and collaboration with the care team are essential to success.

Qualifications

• Active, unrestricted Massachusetts NP license.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, or dementia settings.
• Strong clinical judgement and documentation skills.

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Develop care plans and manage ongoing treatment with supervising physicians.
• Partner with neurologists and care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans and disease progression.
• Maintain accurate clinical documentation in the EMR in compliance with regulations.
