Zusammenfassung

Local Infusion is seeking a Nurse Practitioner licensed in Maine to join our virtual care team in a remote, patient-facing role delivering care to Local Infusion patients and partnering with physicians.

You will assess, treat, educate and document via telehealth, collaborating with neurologists to improve access and time to treatment, while enjoying a supportive, growth-oriented, remote-first environment.

Qualifikationen

• Active, unrestricted NP license in Maine
• Certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years clinical experience in neurology, memory care, and/or dementia care
• Strong clinical judgement and clinical documentation skills
• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication with patients facing cognitive decline and their caregivers

Aufgaben

• Provide direct clinical care to patients with neurological conditions via telehealth
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies
• Partner with our neurologist(s) and broader care team to ensure continuity of care
• Educate patients and families on diagnoses, treatment plans, and disease progression
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
