Qualifications

• Must reside in the State of Arizona.
• Ownership and results driven.
• Ability to effectively plan, prioritize, organize time and workload.

Responsibilities

• Provide one-on-one phone/video consultations with Medicare Part D members.
• Consult with patients to identify, resolve, and prevent medication-related problems.
• Prepare a medication summary and personal care plan with recommendations.

Skills

Arizona Pharmacist License in good standing

Excellent verbal and written communication skills

Education
