Job summary

Pharmacy Careers is seeking an MTM Pharmacist to join our growing team. Conduct comprehensive medication reviews, coach patients on adherence, and collaborate with prescribers to optimize therapy, often from home or with flexible schedules.

Education requirements include PharmD or Bachelor of Pharmacy. All licensed pharmacists are encouraged to apply, with ambulatory, hospital, or community MTM experience a plus.

Qualifications

• Education: PharmD or Bachelor of Pharmacy is required.
• Ambulatory, hospital, community MTM experience is a plus; licensed pharmacists welcome including new graduates.
• Strong patient counseling, documentation, and collaboration with care teams.

Responsibilities

• Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
• Identify medication-related problems, interactions, adherence barriers, and cost-saving opportunities.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM program compliance and quality metrics.
