Job summary

CVS Health Corporation is seeking a Pharmacist for Medication Therapy Management (MTM). After completing a 90‑day training, the role becomes 100% remote from home, with onsite training at 4750 South 44th Place Phoenix, AZ 85040.

The pharmacist will deliver MTM services via phone/video and collaborate with the care team to optimize patient outcomes. Requirements include an Arizona Pharmacist License, PharmD degree, and strong communication and organizational skills.

Qualifications

• Arizona Pharmacist License in good standing.
• Must reside in the State of Arizona.

Responsibilities

• Provide MTM services via phone/video to commercial and Medicare Part D members.
• Collaborate with care team and program managers.
• Assist with intake/enrollment processes when needed.
• Assess medications for indication, effectiveness, safety and adherence; use lab data when available.
• Prepare medication summaries and personal care plans for patients.
• Follow up with patients to monitor drug therapy and outcomes.
