Job summary

Rising Ground is seeking a Medical Support Professional to provide person-centered medical, clinical, and behavioral care for residents in New York. The role includes administering medications, coordinating appointments, and maintaining accurate health records in a supportive team environment.

Immediate start, full-time hours, with a schedule in the Westchester & NYC area. Must meet state requirements, pass background checks, and complete trainings such as AMAP, SCIP, CPR, and First Aid.

Qualifications

• HS diploma or equivalent required.
• Preferred six months experience with individuals with I/DD.
• Must pass Criminal Background Check and Fingerprinting; must complete mandatory trainings (AMAP, SCIP, CPR, First Aid).

Responsibilities

• Provide medical, clinical, behavioral, and safety support to residents.
• Maintain NYS certifications to administer medications including insulin and provide first aid.
• Schedule and accompany residents to medical appointments; document follow-ups.
• Maintain medication records and inventories; coordinate reordering and pharmacy pickups.
• Monitor residents' medical conditions; communicate changes to Nursing and follow clinical recommendations.
• Respond to medical emergencies and obtain necessary treatment per agency policies.
